Primary, fork, and sideband Live lanes now retain only messages whose send was never attempted. Delivery-uncertain sends are reported as errors instead of being replayed or returned as unsent.
fix(responses): reserve lane IDs across WebSocket session replacements openai/openai-node
Closing and replacing a managed Responses session on an open WebSocket could let a delayed result from the old session complete a new request. Lane IDs now stay reserved for the physical socket's lifetime.
Retain confirmed Code Mode messages for Guardian reviews openai/codex
Guardian reviews need the assistant's delivered question to interpret a user's reply, so confirmed Code Mode messages now persist as review context. Approvals based on earlier context go stale when a new request arrives.
