Tool, file-write, and terminal questions now go through the existing shared prompt helper, and the queue holds ownership until the actual question finishes, even when a helper signal cancels a waiting caller.

Repeating top-level `--cwd` loaded configuration from the first directory while Commander executed in the last, so an approve-all first project could authorize writes inside a final deny-all project; repeated `--mcp-config` had the same first-value mismatch. Bootstrap paths now share one last-value scan, and configuration errors route through the normal output formatter.

Agent names that collide with JavaScript prototype properties broke CLI and embedding resolution: `constructor` could return the inherited `Object` function, and JSON-configured `__proto__` entries vanished during normalization. The built-process agents E2E matrix drops from 45 cases to 13 while still exercising human, JSON, and dual-TTY finalizers: 65.600s down to 31.716s on the same checkout and built artifacts. The companion sessions matrix goes from 45 cases to 9, 62.173s to 14.844s, keeping route-first and Commander failures plus the operational exporter failure path.
